1. Each participant must have an OLMC sports registration form completed.
2. Each parent must sign a parent consent form.
3. Every player participating in a CYO sanctioned sport (grades 3-8) must complete a sport physical form.
4. All forms must be completed and turned into the sports office (via the parish or school office) by the given deadline.
5. Payment is due along with the registration form. Make all checks payable to OLMC Sports. |
